DESCRIPTION:The Pathways Community HUB offers an innovative approach to implementing and financing coordination services that address the non-medical drivers of health. This system empowers Community Health Workers to assess patient needs and facilitate their connection services. The Bexar County Community Health Collaborative will share its experience working with health plans and providers to meet patient needs.
