DESCRIPTION:CommUnityCare\, one of Texas’s largest Federally Qualified Health Centers (FQHCs)\, served nearly 130\,000 patients. This webinar examined the organization’s experience developing a holistic\, closed-loop referral process to address patients’ non-medical needs. Participants learned how CommUnityCare built community engagement strategies to maximize uptake among patients\, staff\, and community-based organizations.
