SUMMARY:Bridging Medical and Social Care: University Health and Family Services
DESCRIPTION:University Health provided its patients with tailored NMDOH case management through a partnership with Family Service\, a community-based organization with more than 120 years of experience addressing the non-medical drivers that influence long-term health and life potential. Family Service was integrated into the University Health electronic health record system\, enabling seamless referrals\, coordinated care\, and ongoing follow-up. This session highlighted how the model strengthened care planning and advanced equitable access to health care by addressing the underlying factors that shape well-being. Early results showed that\, since its launch in October 2023\, the program had served more than 160 patients and initiated over 170 NMDOH pathways. \n\n\n\nClick to view the presentation slides\, here.
