On April 22, 2025, this panel from the University Health and Family Services in San Antonio described their partnership to address the non-medical drivers of health (NMDOH). They shared how Family Services is integrated into the University Health electronic health record system, enabling seamless patient referrals, coordinated care, and follow-up. This session describes how the model strengthens care planning and promotes fair access to health care by addressing the underlying factors that impact well-being.
