On March 27, 2025, this panel from the CommUnityCare Health Centers, one of Texas’s largest Federally Qualified Health Centers (FQHC), shared their experience building a holistic approach in implementing a closed-loop referral process for addressing patients’ non-medical needs. CommUnityCare developed community engagement strategies to maximize uptake among patients, staff, and community-based organizations. This panel highlights their approach for workflow development, training, and dissemination.
