With health-related, non-medical needs having a greater influence on health than medical care alone, identifying and addressing the social determinants of health (SDOH) is crucial to improving outcomes, especially in value-based payment systems. This article outlines how to screen for SDOH, connect patients with necessary resources, and incorporate this system into the workflow. Additionally, it suggests that ICD-10 “Z” codes can be used to document SDOH and give practices accurate data on the needs of their patients. Ultimately, systematically accounting for SDOH is a promising avenue to truly supporting patient populations.
