Bridging Medical and Social Care: University Health and Family Services

University Health provided its patients with tailored NMDOH case management through a partnership with Family Service, a community-based organization with more than 120 years of experience addressing the non-medical drivers that influence long-term health and life potential. Family Service was integrated into the University Health electronic health record system, enabling seamless referrals, coordinated care, and ongoing follow-up. This session highlighted how the model strengthened care planning and advanced equitable access to health care by addressing the underlying factors that shape well-being. Early results showed that, since its launch in October 2023, the program had served more than 160 patients and initiated over 170 NMDOH pathways.
