Implementing a Closed Loop Social Care Referral Platform to Address NMDOH

CommUnityCare, one of Texas’s largest Federally Qualified Health Centers (FQHCs), served nearly 130,000 patients. This webinar examined the organization’s experience developing a holistic, closed-loop referral process to address patients’ non-medical needs. Participants learned how CommUnityCare built community engagement strategies to maximize uptake among patients, staff, and community-based organizations. The session also explored the development of workflows, training approaches, and dissemination strategies that supported successful implementation.
