Title: HHS Strategic Approach to Addressing Social Determinants of Health: Agency Program, Activity, and Policy Highlights

This HHS Strategic Approach report highlights key programs, activities, and policies, including early accomplishments and personal stories of beneficiaries. The activities are organized into three goals: enhancing data infrastructure, improving connections between health and social service providers; and engaging in whole-of-government collaborations to implement comprehensive solutions. Together, these actions encourage health care providers to screen and address HRSNs through increased coordination of care, partnerships, payment, and data collection. Lastly, this report identifies illustrative examples of programs, activities, and policies related to each of the Healthy People 2030 SDOH Domain.
