Using Risk Stratification to Identify and Address NMDOH Needs in Rural Communities

Burke is the Local Mental Health Authority (LMHA) for eastern Texas, managing a census with a high prevalence of complex health needs. This session outlined the risk stratification process developed to strengthen service delivery within two rural outpatient primary care clinics. The approach emphasized precision in identifying patient risk levels and needs, including NMDOH, to ensure limited resources and staffing were allocated effectively and strategically.
