Bridging Medical and Social Care: University Health and Family Services

University Health provides its patients with tailored NMDOH case management. This is made possible through a partnership with Family Services, a community-based organization with over 120 years of experience addressing NMDOH that impacts long-term health and life potential. Family Service is integrated into the University Health electronic health record system, enabling seamless patient referrals, coordinated care, and follow-up. Join this session to learn how this model improves care planning and fair access to health care by addressing the underlying factors that impact well-being. In addition, early results indicate service to over 160 patients, and initiation of 170+ NMDOH pathways since its launch in October 2023.
